{"id":216,"date":"2018-09-10T20:49:39","date_gmt":"2018-09-10T20:49:39","guid":{"rendered":"https:\/\/caminoent.com\/patient-resources\/what-to-bring-to-your-appointment\/"},"modified":"2018-09-19T16:20:30","modified_gmt":"2018-09-19T23:20:30","slug":"what-to-bring-to-your-appointment","status":"publish","type":"page","link":"https:\/\/caminoent.com\/patient-resources\/what-to-bring-to-your-appointment\/","title":{"rendered":"What to Bring to your Appointment"},"content":{"rendered":"<p><strong>Please bring:<\/strong><\/p>\n<ul>\n<li>A photo ID<\/li>\n<li>Your current insurance membership card<\/li>\n<li>A credit card<\/li>\n<li>A referral or authorization number (if required for the visit)<\/li>\n<li>A list of all medications you are currently taking<\/li>\n<li>All relevant film or scans and their corresponding interpretation reports<\/li>\n<li>Any relevant prior medical\/surgical\/audiological records<\/li>\n<\/ul>\n<p>If you had an audiogram performed in the past and your appointment is for an ear related issue (including dizziness\/vertigo), please bring a copy of the report with you to your visit.. If you do not bring a copy of the report or the results are older than 6 months, you may be asked to have a repeat audiogram.<\/p>\n<p>If your appointment involves cognitive or memory issues, we suggest you bring a family member or caregiver with you. This individual should be familiar with your background information and your condition, in order to assist the physician. If the patient is coming from a care facility (e.g., skilled nursing, assisted living, memory care), we ask that a staff member be present for the entirety of the appointment, if a family member or friend cannot be present, to ensure the physical and emotional well-being of the patient.<\/p>\n<p>If you have limited or no English-speaking proficiency, we ask that you bring a family member or friend who can interpret for you. If you speak American Sign Language (ASL) and would like an interpreter to be provided for your appointment, please notify us prior to the day of your appointment so we can arrange for this service.<\/p>\n<p>To speed up the check in process, we recommend all patients print out the new patient registration forms, complete them at home and bring the completed forms to their appointment.
