Pet Policy
We happily welcome service animals in our clinic.
Please refrain from bringing your pet to the clinic. Only service animals are allowed; per the ADA, they must be tethered, leashed or harnessed and housebroken.
What is a service animal?
Service animals are not pets. They are trained to provide assistance to people with disabilities, improving their mobility and independence. We do not require proof that they have been trained as a service animal.
Why are service animals welcome?
It is our goal to create a welcoming environment for all our patients and to enrich the lives of those who require special assistance.

* Congress passed the Americans with Disabilities Act (ADA) in 1990 to prevent discrimination against people with disabilities. The law grants all people with disabilities the legal right to be accompanied by a service animal in all areas open to the general public.
Consent for Minors
A parent/legal guardian of the minor must be present at the initial visit to sign financial, privacy and safety agreements. Please fill out the Consent for Minors form and bring to the initial appointment.
