Dr. Murray accepts CCS patients for the Children’s Hospital of Oakland Cochlear Implant Program. These patients can be seen at both the CHO clinic and here in our San Jose and Morgan Hill clinics. We are not able to see other Medi-Cal and or QBM (Qualified Medicare Beneficiaries) patients at any of our locations as we are not in-network. We are also unable to accept Medi-Cal or QBM as a secondary payer on other plans. Therefore, if you have either Medi-Cal or QBM, we are unable to see you as a patient.
Union Sponsored Health Care Plans:
Most union sponsored healthcare plans participate through regular PPO networks, so we are able to see most patients. However, we recommend checking with your union plan before scheduling an appointment.
All commercial PPO plans including:
Blue Cross: We accept all major Blue Cross PPO Plans with the exception of Pathway PPO/EPO and Blue Cross Select Plans. Please note: Pathway plans do not cover California patients. This also includes plans in remote areas and patients who may not have had insurance prior to the mandate. If you’re uncertain about your plan, please call your insurance directly for assistance.
Blue Shield: We accept all major Blue Shield PPO plans.
AETNA: We accept all Aetna PPO plans with the exception of Student Health Service and Stanford Health Alliance (SHCA). If your Aetna plan is a managed choice or select plan you will need an electronic referral from your PCP (primary care physician) prior to being seen at our clinic

UnitedHealthCare: We accept all United HealthCare PPO plans with the exception for United Healthcare CORE plan.
HealthNet: We accept all HealthNet PPO and POS plans for all tiers. For Tier 1 plans, the administrator must be SCCIPA and or Caremore.
HMO Insurance Plans:
SCCIPA/ Caremore: All providers are in-network with SCCIPA and Caremore HMO plans
Tricare/ Triwest: All providers are in-network with Tricare and TriWest with the exception of Dr. Mark Kita. He is currently not in-network with TriWest.
